This page is for individuals and
families who have already submitted their donation of $200 to the Friends of Beacon Dog
Park.
If you have not yet
donated,

Don't be alarmed if the forms below mention
anything about payments, you've already paid and we have records of
that.
Just follow the instructions
below:
Steps:
- Use the form below to enter your personal information
if you would
like an icon/clip-art on your brick be sure to select "yes" and indicate the clip-art# from
the list to the right.
- Click continue, the next page will allow you enter the text you would
like to appear along with the specific clip art you selected
above.
- Important: you have already paid for the brick, so
just enter the "0" into the "Check Number" field.
- We will review your order and contact you if
required.
